Case Management

Reaching The Plateau . . .

Medical and Vocational Case Management are essential tools in handling of workers' compensation, disability, and liability claims. Through monitoring and coordinating medical treatments and early vocational intervention, carriers and employers can benefit from lower medical costs and reduced lost time. Injured workers benefit from swift medical treatments, an objective liaison to assure quality care, and a return to productive and gainful employment.

CCM's staff is trained in the assessment and coordination of medical and rehabilitation needs. This is accomplished through contacts with the injured worker, family members, treating physicians and other health care providers, employers, and carrier. The carrier and/or employers are kept apprised of medical status and anticipated length of disability for a Return To Work Plan.

The primary goal of carrier, employer, and employee is to return the injured worker to work as soon as possible. Sound and swift return to work planning improves the employee's quality of life and minimizes employer and carrier costs. Case Management has two parts:

Medical Case Management:

Medical care management ensures appropriate care at an appropriate price is provided. Case managers are trained, experienced, and certified professionals who work closely with the carrier, employer, health care providers, and injured workers to:

  • Maintain contact with treating physicians regarding treatment plan status.
  • Keep the carrier and employer apprized of the injured worker and case status.
  • Medical coordination between physicians, rehabilitation specialists, surgeons, etc. for continuity of medical care.
  • When maximum medical improvement is reached, coordinate a return to full duty work or develop a transitional return-to-work plan.
  • Vocational Case Management:

Case management does not necessarily end when medical treatment ceases or maximum medical improvement [MMI] is reached. At times, an employee FAILS to return to their preinjury job. CCM will provide the appropriate level of required vocational services to return the employee to gainful employment through:
